You’ll have to excuse my ignorance but what do people mean when they say they have trained their car?

I assume that if you put your foot down the car will give everything it’s got?

It would be dangerous to only give mediocre acceleration because someone typically drives slowly.

I'm not sure I know what you mean by "train their car" As far as putting your foot down and the vehicle giving it all it has, from the factory, a "delay" (for lack of a better term) is written in the [stock] programing creating a slight lag. This is one of the parameters a "tuner" will address (along with bost, timing and fuel curves) to optimize performance. It's not unusual to see a gain of 10 to 15% in overall performance.

I installed a peddlemax on my BB (the same one I used in my 16 Focus and 18 EcoSport, gotta love "plug-and-play"). This unit eliminated the lag in throttle response. It did take a few moments to get used to, the difference was night and day. So I suppose I had to "train my foot", not the car

I'm not sure about any horsepower or torque gains, I haven't put it on a dyno. However I can tell you there is a huge difference in "seat of the pants" feel on better acceleration and for whatever reason, I gained 2 mpg overall.
