Any ideas on how to fix this or is this a known issue?

Not a known issue that I have seen or read about here.

I'd start by visually tracing the water tubes from the fluid tank to the nozzles for leak.

Here is my reply to a similar question from Feb. 21, 2024.
The post title was “Front Camera Built-in Washer”
————————
I just had my dealer replace the washer assembly, mere weeks before my factory warranty was to expire.

Q: Did it solve the leak?

Yes. But the Ford Service scratched my grill in 3 places and 2 paint chips at front fender. But no more “Fluid Low’ warnings. I had the warning appear on my way home after purchase. I checked the fluid level and it was near empty. Also my Coolant reservoir was empty, so I blamed both problems on the dealer, for poor pre-sale preparation. So I filled both fluids, but the Washer reservoir emptied that same day. I saw the puddle on the ground and knew immediately what the issue was.

This has started happening to mine more and more these past few months. I'm over 60k already, so no longer under warranty. Did they have any specifics about failed? I would guess that maybe just the washer pump would need to be replaced and not the whole washer assembly?

Mine is doing the same. I'm thinking I will just block the line going to the camera. I'm out of warranty and can live with manually cleaning the camera once in a while.
